1. My provider numbers are not correct...Go to Office Setup on the Maint Menu.  The default provider should be the code for one of your providers.  You may leave this field blank.  If the default provider is blank, uncheck the "Use Default Provider for all Claims".  The office federal tax id number will be printed when the provider cannot be located.  You may edit the INS LAYOUT, under the MAINT MENU, to add the PRO:SPECIALITY and the code ZZ to the claim. Some commerical claims require taxonomy code in box33 b.

2. The providers Medicare number is not printing...Go to the Insurance Companies, select the Medicare Carrier, click on the change button.  There is a box in the lower right corner, Type.  Check the radio button for Medicare.  The provider numbers are tied to the insurance carrier by type.  For Blue Cross/Blue Shield, check the BCBS.  The Provider update screen contains space for common provider numbers.  Provider and Group numbers may be entered for any insurance carrier.

3. The information on the insurance is not printing in the proper boxes.... Go to the Ins Layout on the Maint Menu.  The Insurance Layout screen allows you to change the layout and content to be printed on the insurance form. The program is shipped with three layouts, HCFA, UB92 and UB92L. You may save the default HCFA by changing the name to HCFAOLD.  A Copy Form button is provider to copy a form layout.  The Copy Form screen allows you to globally add or subtract a row or column from all lines.  Enter 1 to move all Rows down or -1 to mover all Rows up. You may edit the individual line items by changing the Row or Col numbers. The font used on the HCFA and UB92 is the 'Courier New' font.  You may need to download or install this font to your printer.  The HCFA printer (generic text only) will not print a 12 characters per inch.  You will need to use your default printer or leave the printer name blank on the insurance layout screen.  You may call for free advice on setting up an insurance printer.

4. I want to always print 'Signature On File' in Box 12....See item 3, above. Go to the Ins Layout on the Maint Menu. Select Change to change the insurance form layout.  Find Box 12 on the right hand side of the table.  Delete the text in the FIELD column and add any text you want printed in the DEFAULT field.  Read the help screen.

5. Where is the users manual?...Go to the Windows Start Button -> Programs->MedicalBilling->Users Manual. This is an Adobe file.  You will need Adobe Acrobat Reader,.    

6. How can I enter a Group Number for an Insurance Company?...Go to the Utility Menu, Providers. Locate your provider, select the Change Button, go to the Provider Numbers Tab.  Provider numbers are tied to the Insurance Company and Provider.  Select Insert to add a new provider number.  Select the Insurance Company and enter the provider and group numbers.  Provider numbers on this screen will override provider numbers on the main Provider update screen.

7. I am receiving a error message 'Nothing to Preview' or 'Nothing to Print' when attempting to print insurance forms.... There are many causes for this. The program attempts to ensure that any insurance claim be as accurate as possible.  Check Patient Update screen, there is a check box on page 3 that disables printing for one patient.  There is a check box on the Insurance Company update screen to disable printing claims for a carrier.  The Insurance Company must have a FORM NAME. The FORM NAME is the name of the Ins Layout form that the carrier uses.  HCFA is the default 1500 insurance layout.  You may print claims by date entered (DATE) or date of service (DOS). Make sure that the date type matches the date range.  Each charge line may be disabled from printing on the claim.  Check the FORM screen when on a charge line. There is a check box that disables printing a line item. Try printing the claim for one patient by going to the patient Accounts screen, hi-light the charge line (one with 'C' in column 1), select the F9 Print Ins button.  You should be able to print any single patients insurance claim, unless the line items are disabled.  Be alert to the ALL INSURANCE, ELECTRONIC and NON ELECTRONIC setting.  The patient must have a primary insurance (insurance number 1) to print the primary carrier and needs a secondary insurance (insurance other then 1) to print secondary insurances.

8. How do I obtain support?... Email support is free and the best method. You may purchase support by the incident, quarter, by six months or yearly.  I try to answer short well defined questions via phone for free.  I will return calls to users who have purchased the program but will not return calls to free users.  Support may be ordered on the Order Page.

9. How do I receive updates, notification of problems?..You should have received a login and password. The login and password must be in lower case. I send login instructions when you complete the registration form on the download page.  I do not routinely send emails.  You will be notified of major problems.  You can opt out of any notification by entering 'OPT OUT' or anything similar in the address field of the registration form.  You may send me email stating that you wish to receive no further emails.  Check the NEWS page

10. Will the price increase?.. Probably.  The price quoted on the order page is the official price.  I will honor this price for 30 days from the date of your initial download, or the date I ship your CD. 
 
11. I need CPT codes, ICD9 Codes, the Type of Service and Place of Service Codes seem incorrect.... CPT codes are copyrighted by the AMA.  I can convert your CPT codes for you. There may be a small charge for converting the file.  I will need a letter or email stating that you own the CPT file.  The ICD9 file, Type of Service, Place of Service and all other codes are your responsibility.  These codes are constantly changing. Part of setting up the program is checking and entering correct codes.  I can convert data from many other billing programs.

12. How can I test the program, is there any sample data? There is a sample data directory.  Goto the Start Button, select Programs, locate Medical Billing, Click on Sample Medical Billing.  Use dates of October 2000 to test the reports.

13. I need to be able to assign chart numbers. The patient demographic Billing Information Tab contains a field where you enter your Chart Id.  You may also locate the patient by Chart id.

14. Unable to locate C:\WINDOWS\TEMP\IFORM.TPS. The med.ini file has an incorrect temppath variable.  Windows 2000 and NT do not have a windows temp directory.  Change the TEMPPATH=C:\WINDOWS\TEMP to C:\WINNT\TEMP or create a C:\TEMP and change temp path to TEMPPATH=C:\TEMP.  The temporary directory must be present.

15. My screen size/position is off the screen. It is possible for you to change the size and position of the screens. C:\windows\tmed.ini contains the position and size of all screens.  Delete this file, the screens will all be restored to their default size and position.

16. Medigap/secondary insurance information is not printing for Medicare claims. You must be a participating provider.  Check the PARTICIPATE check box on the Office Setup screen.

17. My accounts show posted, the Insert and delete buttons are missing on the enter charges and payments screens.  Someone has ran a day sheet posting report.  This marks the day as posted, no charges are payments can be added or deleted.  You may un-post the day.  See the on-line help for information on un-posting.

18 How do I print more then 2 modifiers on the 1500?  The sample Insurance Layout did not include spaces for printing Modifier 3 and 4.  You enter modifier 3 and from from the charges screen.  Hi-light the charge line and select the "F5 Use Form" button.  You will need to modify the Insurance Layout.  Go to the Utility menu, select INS LAYOUT, Modify the layout to add IFO:MODx[3] and IFO:MODx[4], where x is between 1 and 6 (for the 6 lines on the 1500.
