01/30/2010, Read the newsletter.pdf, we have changed the update policy.  One time
update CD's or Download will cost $30.00, a yearly update subscription is $60.00.
 
01/24/2010, Instructional vidoes have been added to the download page, this is a 54
megabyte file, very large.  Order a CD if you have trouble downloading the videos.  There
will be a change to obtaining updates, a new newsletter will be ready in a few days.
 
12/16/2009, Minor changes to license agreement.  Instructional videos are not included
with a CD or Program order.
 
10/14/2009, There was a missing DLL in the 10/13 download, this haa  been fixed, sorry.
 
10/13/2009.  Added a YTD aging report to LINEPAY.  Shows the aging data by month
from January 1st to the cutoff date.
 
09/18/2009 The med.ini in the file dated 9/14 has a problem.  Remove the HPATH=C:\medbill\history or download the new SETUP.EXE, dated 9/18
 
09/14/2009, User manual updates, Linepay.pdf and linepay.hlp. The History program along with a new History manual and help file has been added. More manual updates will be ready in a few weeks.
 
08/03/2009 If you are missing the date of service to (ending) date, you may edit the Ins Layout from the Maint Manu or delete mreport.tps and forms.tps, download the new setup file from the download page at www.billingmedical.com. Free phone support will be provided for this problem.
 
07/08/2009, More updates to the users manual and a major update to MED.HLP.  More updates will be coming.  MED EMR users if you are having trouble with understanding or using the templates:  Delete tform.tps and templates.tps, insall the latest update.  This will give you better starting templates.
06/29/2009, The users manual for MED, MANUAL.PDF has been updated.  We will be updating
the help files and all other user manuals.

05/23/2009, Changes to the Visits report, added summary for providers and insurance carriers. Many updates to the Medical EMR program.  You may print a prescription script using Microsoft Word mail merge.  Using MS Word will allow you print, email or fax the prescription.

01/14/2009, Add EIN/SSN for Molina Healthcare and other Option 3 NPI carriers. Remove
SSN/EIN from referring physicians if NPI is present.
 
12/20/2008, Fix for delete of payments, prior to this the actual line deleted may have been wrong.  Enable provider  name change on payment screen.  This is a short test.  we may return to not allowing provider name changes on the payment screen. There has been one report of problems in entering payments. Let me know if you are having problems with entering payment line items.
 
11/26/2008, Ambulance changes, add city, state and zip. These fields are needed for the
point of pickup for medicare.

 
11/03/2008, The user manual and help files are not complete for the Version 1.0 of MEDEMR. The new setup.exe  contains the MED and MEDEMR program.  The user manuals are contained in EMRMED.ZIP, both DOC and PDF versions. The supervising phyisician field on the charge header should not be used unless you have a supervising provider.  Use the Referring Physician field.
 
08/19/2008, Some insurance carriers will not accept federal id numbers in the ANSI file. There is a new check box on the Insurance demographic screen, Do not use federal id.  Checking this box will not print the federal id number within the  ANSI file.
 
07/28/2008 Some providers have had trouble with being forced to use SSN numbers for edi claims sent to Medicare.  The Employee ID Number (Federal ID) is needed by other carriers.  Leave the Federal Id number on the Provider Demographic screen.  Add a new section to the Med.ini file. See the news and info page for exact instructions.
 
Add these lines to the Med.ini if you use SSN for Medicare Federal ID for others.

07/03/2008, You may need to remove your legacy id numbers from the Provider demographic screen and the Provider number tab screen. 
 
07.02/2008, Change Vista padding to correct clipped text:
1. Open Control Panel
2. Open Appearance and Personalization
3. Click on Personalization
4. In Personalization, select Window Color and Apperance
5. Click on Open Classic Appearance properties.
6. Select Window Aero Scheme, click Advanced
7. In Item  Dropdown list, select Border Padding.
8. Set the Border Padding to 0 or 1
9. Save your changes.
 
07/01/2008, Support for 10 digit zip codes has been added to the ANSI file.  All 10 digit codes may be used and will be added to the ANSI 837 file.
 
06/08/2008.  Added loop 2310E to the Ansi file.  This is needed for certain cpt/diag code combinations.  Missing loop 2310E will cause a CN1 error from Medicare.
 
06/03/2007, Added support for simple ambulance billing.  A screen has been added to the charge header screen.  You must have a place of service code equal to 41.
 
05/30/2008, NPI numbers are required in box32, but some service locations are not required to have an npi, SSN, FEDID numbers cannot be used.  The program will not force an NPI in box32.  Enter the npi's for the facilities that require one, leave it blank for other service locations.  You may need to delete some provider numbers or group numbers on the provider screen.  You may need to edit or create a new insurance layout for a specific carrier.
 
05/28/2008,  Changed the payment screen to accept a Modifier 1, This may be needed to identify the charge line when the same cpt code is used on the date of service.  If you have a Medciare Group id number, delete this number on the Provider number tab.  You may need to remove some provider numbers on the Provider demographic or provider number tab.  Some carriers do not want the legacy id number some do.
 
04/03/2008  A new program MEDEMR.EXE has been added to the MED setup.exe file.  This is the start of an integrated Electronic Medical Record.  I do not have the help file or user manual ready for the EMR.   The final program will cost in the $300-400 range.
 
03/28/2008, Change to Reimbursement Report, add summary by cpt and payer. Fixed CPT Increase by percent.  Added security to History program.
 
01/30/2008, Direct filers to ArkMedicare, vendor number is V02006.  The users manual will be updated with instructions for the ISA02 field. The Medicare legacy ID numbers, 1C Medicare and 1G Upin are no longer required on electronic claims.  the new SETUP.EXE will remove these numbers from the ansi file for the MED program.  The CLAIMS program has been  updated.  I am not sure of the requirement for the Legacy id numbers on paper claims. 
 
01/18/2008 Install new SETUP.EXE, received a report of possible corruption in the setup file. No other changes, just recreated the setup.exe file.  Please report any insurance layout problems.  No charge for phone support on this issue.
 
01/06/2008, Changed Linepay.exe to update the payment facility with the charge facility. Add new option on the Maint Menu, Increase CPT charge by percentage. Use the same password as the Unpost option.
 
12/06/2007, Fixed ansifile.tps name length.  There was a mis-match between ansifile.tps and forms.tps.  Ansifile.tps is used for unique ansi settings, usually ISA and GS record.
 
12/02/2007.  Fixed MSP, Medicare Secondary Payment for EDI filing. The file passes initial edit checks, but there is no guarantee that payment will be made.  The prior version also passed edit checks.
 
11/19/2007 Fixed a problem with HCFA 1500 Box30, sometimes the cents would not appear.  Added IFO:DOS6[x] and IFO:DOSF6[x] to the
insurance layout.  You may change the IFO:DOS[x] to the New IFO:DOS6[x], IFO:DOS[x] prints MMDDCCYY, IFO:DOS6[x] prints MM DD YY.  You must change all, DOS and DOSF to the same type, either IFO:DOS[x] or IFO:DOS6[x] and IFO:DOSf[x] or IFO:DOSF6[X].
 
10/18/2007, Changed UB04 EDI file to use Patient insurance file if patient and subscriber are not the same person. This does not change the paper claim.
 
10/09/2007, Data from the patient special field will appear on the update appointment screen,
 
10/02/2007, Fixed a problem with UB04 and creation of the Ansi edi file for UB04.
Changed delimiter for HI *B? fields. Some users were having trouble with extra date fields.
 
08/01/2007, Fixed UB04 printing from the MED program.  We are testing the UB04 electronic filing, passed with MediCal, Should have Availity, Navimed and THIN testing completed by mid month.
 
07/15/2007, Update to MED, change to ansi file.  The office npi number will be used in box33, if it is present on the office setup screen.  Some carriers want the provider taxonomy code in box33 b, use pro:speciality, some want the legacy provider group number, use ifo:grp.  Always check the iform.tps file, use topscan.exe to open c:\windows\temp\iform.tps or where ever you are putting your iform.tps file.  The manual.pdf file has a list valid field names.  You may put any field anywhere your carrier requires.  The EDI ansi file usually is always correct, no matter what fields are on the HCFA. 
 
07/13/2007  Claim notes has been added to the 1500 and UB04 browse screens.  Enter as many notes as you wish.
 
06/20/2007, Fixed claims program box 39, 40 and 41 amout fields.  These were changed from a dollar amount to a string field.  You may enter any data in the amount field. Fixed box 55b and c.
 
06/16/2007, Added fields, 57a-c, 67 9-12, 81.  UFO;BOX56A is the npi.  Use UBLASER.EXE to add the new fields and create a new UB04L layout.  You will need to use UB04L on page 5 of the UB92 to use the new layout.  You will need to tweak the column numbers in the UB04L and UB04 for the new fields.  Backup your data prior to installation.
 
06/08/2007, I have increased the defaul line length, You may select a Courier font 10 pitch, this will allow 12 characters per inch.  You will need to change the INS LAYOUT to move your data to the proper columns, normal layout is 10 characters per inch.  I may add a UB04L for laser layout with a import feature, depends on interest.
 
06/02/2007, Added field 3B and allowed changes to line 23 box45, creation date, you are responsible for the creation date.  Backup your data, this update will increase the size of box80 from 20 to 24.  You may need to change some column numbers on the UB04 layout.  Email help is available.
 
05/08/2007, The update for UB04 and Claims is ready.  Electronic filing for UB04 will be ready in a few days.  Upgraders, you must follow these instructions.  Failure to follow the instructions will corrupt your data. 
 
Backup your entire \CLAIMS directory, including sub directories.
Download the new Claims setup.exe program.
Run setup.exe, but DO NOT START THE CLAIMS PROGRAM.
Locate and run C:\CLAIMS\NEWUB04.EXE
Start the CLAIMS program
Go to the Maint Menu, Run Convert Data.
 
There will be a charge to help users who do not follow these steps.
 
04/14/2007, Changes to CLAIMS program.  Fixed the convert data function to change the dates to the new format.  Upgraders should run  Convert data, under the MAINT menu.  This will change your dates and check boxes. Run newform.exe and check the NEW FORM check box to use the new 1500 INS LAYOUT.
 
03/30/2007, Dates in box24 have been changed to full mmddccyy, 8 digits for paper claims. The user manual (manual.pdf) has been updated to include files and fields for the insurance format setup.  You may use IFO:DOS6[X] AND IFO:DOSF6[X] if you want to use the mm dd yy  format on your paper claim. For Medicare, enter only one diagnosis pointer code in box 24e. Visit a CMS site, read the instructions for the new CMS-1500 form.  These differ from the www.nucc.org instructions.
 
02/21/2007, The users manual, Manual.doc and Manual.pdf have been updated to included instructions for Availity Electronic filing.  You need to add fields to the Ansi Tab of the INS LAYOUT screen and edit the MED.INI file.  See page 104 of the manual.
 
02/18/2007, New CLAIMS program.  Uses the new 1500 layout.  Use NEWFORM.EXE to update your HCFA Insurance Layout.  Backup your complete CLAIMS directory, run the new SETUP.EXE, backup the complete directory then run NEWFORM.  This is a major update to the CLAIMS program.
 
01/18/2007, Added the ability to see appointments for 1 provider for 15 days.
 
01/12/2007, Changes to the ANSI EDI file.  Avality requires one ISA/IE section per physicial file.  The Claims program is almost ready.  All MED users may download and use the CLAIMS program.
 
01/07/2007.  You may print the new insurance form and data.  Set HCFA=RED in the med.ini file.  Read the setup.txt file for instructions.  There will be changes to the THIN edi files and the user manual is being updated.  The claims program is being changed to use the same data files as the MED program.
 
01/01/2007, Added newform.exe.  Newform.exe will add the new fields needed for the new 1500 form.  The next update will add a connection between the MED and CLAIMS program.  I am moving print support for the claim and data to the CLAIMS program.  Backup your data prior to installing this update.  Only run newform.exe once. 
 
12/16/2006, Major change for NPI.  Support has been added for the new insurance form and for NPI on electronic claims.  This is a major update, backup your data.  There will be another update in a couple of weeks.  We will drop support for printing the claim and the data on one form, but will offer the claims program.  Email for support in setting up the new insurance form. Instructions and a small program to automate the changeover will be available soon.
 
08/08/2006, Modifications to ERA print program, added Patient Responsibility.  Changed the allowed amount to pickup the AMT*B6 record.
 
06/27/2006, Added a Referring Physician report to the Reports menu.  You may click on the DOI column of the Charge Header Selction screen to change the sort order to/from assending/descinding.
 
06/21/2006,  New all provider option has been added.  You can use the older all providers or the new menu option.  You can see appointments for up to 12 providers, the top 12 providers with the most appointments for that day.  Use the help button for information on changing colors.  You need MicroSoft Word 97 or higher to print the schedule, print in landscape mode.
 
05/30/2006, Minor change to billing statements.  An account with a current zero balance will not print when using the batch print (A batch print is when the beginning and ending patient name is not the same).  You may always print any single patient statment.
 
05/23/2006, The mail merge file created in the Query program is limited to 51 letters.  You may create a secondary merge file of almost un-limited size.  See the help file in query, search on MERGE.
 
04/20/2006, The ability to create an MicroSoft Word merge file has been added to the
Query search by patient and the Med browse patient and referring physician.  Check the
help file for help on merge.  There is a list of tokens.
 
03/30/2006, Added the ability to use a drop down for cpt codes on the patient payment screen. Add a section to the MED.INI file, [PAYMENTS] with  one entry, DROP=YES.  A button has been added to view account by DOS,  you may mark all charges for a complete day as non printable on a statement.
 
03/13/2006, This is a major update.  National Provider Id (NPI) number fields have been added to the Provider, Referring Physician, Hospital, Facility and Office setup screens.  Backup your data prior to installation.  Install the update on all computers.  The users manual and help screens will be updated in a few days.  You may start using the NPI for Electronic filing.
 
02/27/2006, Minor changes to print Ticket from patient screen.  You can setup a statement form that may be used to print super bills.  The NPI number and updated users manual will be ready by the end of March.
 
12/04/2005, Changes to daysheet and added a referral notice to the accounts browse screen. Add a referral start and end date to page 2 of the patient demographic screen along with the number of visits.  You will see the start, end referral date and total visits used along with the visits authorized.  Users that had data converted may need to run the new 'Clear Referral Date' menu option.  Email for instructions.
 
10/09/2005, Added a Patient Visit report to the report menu.  Shows patient name and the number of visits by day. It is best to use Date of Service (DOS) for this report.
 
10/02/2005, The HISTORY program is being released for use with the MED program.  HISTORY will allow you to move accounts with a zero balance and at least one charge/payment to a history directory.  You may move accounts back to the main program.
 
09/19/2005, Changes for Molina insurance EDI, Enter the new provider id and remove the old group number for Payor ID CIMSA.
 
08/28/2005, The patient insurance number/identifier has been changed from 14 to 16 characters, this is a data base change, so backup your data and program prior to updating. 
 
07/31/2005, THIN submitters, download the new update.  Combined commerical carriers in a single file may have a problem with submitter numbers.
 
07/18/2005, Modified print patient mailing labels to allow selection of one provider; changed unpaid claim report to print a total at end of report and to exclude prior balance.  Change print all provider for one day report. add the section [APPOINTREPORT], item REPORT=YES to print up to six providers for one day.
 
07/05/2005, Price increase for Claims program.  Claims setup.exe contains the correct users manual, Med users manual had been shipped with the Claims program. Minor changes to MED for electronic filing.  Added group check box to proivider screen.
 
06/10/2005, Added optional provider code selection to Patient Balance report.  You may limit the report to one provider, based on charges and payments.  Leave the code blank to get balance based on all providers.  Added sortby=order to [daysheet] setting in med.ini.  You may sort the day sheet by name, ticket number or record order. Caution, record order is not exact, a new charge or payment is added at the first empty slot.
 
05/28/2005, The Claims program will increase in price in early June.  I have added the
ability to print addresses using Micro Soft Word.  The users manual has not been updated, yet. You may print the Patient, Provider and Referring Physician addresses.  There is a new botton on the demographic screens that will open Micro Soft Word with the address ready for printing.  Use the Tools->Lable Envelope option to print the label or envelope.
 
04/27/2005, The user manual and setup.txt had an error in the description of the
[ANSI] RECTYPE.  The correct setting for turning off use of receiver type in the
electronic file is RECTYPE=NO. 
 
04/16/2005.  Timer function added.  You may have the program automatically shutdown
after x amount of time.  See the User manual or look at the Help file, keyword timer. This will only work on 32 bit operating systems, windows 2000 and up.
 
03/07/2005, Some printers would not accept the font change on insurance.  The method used to change the font has been changed.  More printers should accept the font change request.
 
02/25/2004, FTP will not work with anonymous login, so, I have switched to a HTTP transfer.  This should work, but be aware of the file size you receive.  Check the received file size against the size listed on the download page.  I may need to switch hosting services, current hosting is provided by FATCOW.
 
02/22/2005, The default ansi file created is now the 4010 x12 098A, the new version.  You may create the older version by adding a line in the [ANSI] section,  VERSION=OLD.
 
02/17/2005, Minor changes to the Ansi error report for the Claims Program.  Added AMT record for Medi-Cal UB92 out patient ansi file.
 
02/10/2005, The form and data will always print correctly to paper, run the program with the screen maximized and try different percentage views in the preview window, 75% seems to work for most computers.
 
02/08/2005,  When printing the form and data at same time; the data on the screen may appear to be out of alignment.  The printed copy should be ok.  This is caused by different printer/display font settings.  You display may be using a different font from the printer.  You may need to use the INS LAYOUT option on the MAINT Menu to fine tune the insurance layout.
 
02/07/2005, Default settings in MED.INI file have been changed.  The default [INSFORM] setting will print the form and data at the same time.  Remove the [INSFORM] section to print on pre-printed forms.
 
01/31/2005,  Using the Claims or Med program, you may print the data and insurance form at the same time.  Read the setup.txt file or User's manual for instructions.  The MED program will have this capability in a few days.  You will need to modify the INS LAYOUT as to row and cols, but the form will align very nicely.
 
01/09/2005,  Changed the Payments/Adjustment by Type and Ins Chgs/Pay/Adj report  to split amounts by secondary insurance.  The reports do not split andy 3rd, 4th or other insurance. 
 
12/12/2004, Add Special field to patient demographic screen, page 1.  Any text in the special field  will cause the patient name to be hi-ligthed.  Use this to call attention to any special notes or alerts for this patient.
 
10/02/2004, Changes to Ansi 835 report.  Report would crash if you attempted to run an NSF report as an ANSI report.  Increase size of ansi file name to 45 characters.
 
09/12/2004, Price increase.  Ansi 835 era support has been added.
 
06/26/2004, Wrong users manual for MED, loaded new SETUP.EXE containing the correct users manual.
 
06/17/2004, Claims program has been updated, you may now create an Ansi 4010 file for the UB92.
 
06/07/2004.  Changed order of provider id numbers, previous usage was 0B - G2, alpha order.  New order is by primary, secondary, then others.
 
04/08/2004, Added Lab Yes/No and Charge amount for outside lab.  These fields are not mapped to the Ansi EDI format.  Removed Manual.doc from the SETUP.EXE file.  You may need to add the IFO:LABYES and IFO:LABCHARGES to the insurance layout.

08/10/2003, Ansi 4010 x12 addenda, 98 is ready.  Read pages 99 to 102 in the users manual.  The Ansi version has been tested with THIN and is in test with Ok/NM Medicare. 
You must use the new taxonomy codes (speciality codes).
 
05/12/2003, Minor changes to ANSI, Thin requires the file id to be unique to first 6 characters.
 
04/24/2003, Changed the NSF layout to allow both the CLIA number and Prior authorization number to print on the 1500 and be included in the NSF file.  You must add the CLIA number to the OFFICE setup screen.  We are in a state of flux between the new ANSI and older NSF.  I hope to convert all electronic filers to the new ANSI layout.  THIN EDI providers should change to the T0301 NSF layout or the ANSI.  Send an email if you need help.
 
04/20/2003, ANSI 4010 x12 EDI support has been added.  Certified with THIN, The Health Insurance Network, www.thinedi.com .  Read pages 99-101 and SETUP.TXT for instructions.
 
04/12/2002,  Increased the size of the additional Patient notes field from 120 to 200 characters.  Added Medigap Policy check box to patient insurance screen.  First test of ANSI 4010 and THIN T0300 and T060 formats.
 
03/07/2003, The CLAIMS program is missing the office.tps file.  I can send the missing file via email.  LTC customers, do not downland any further updates.  We are changing to the ANSI format and cannot support two NSF formats.  You will be able to update in a few months.
 
02/28/2003, LTC customers must download the new update.  There are changes to the LTC electronic format.  Minor changes, added copay and deduction amounts to patients account screen.
 
02/04/2003,  New FAQ has been added.  You always could use 4 modifiers on the 1500.  The sample INS LAYOUT only used 2 modifiers.  See the FAQ page for instructions.
 
02/03/2003, Minor changes.  Added Patient, Pins, Chghead and Uchghead to the common file.  Added a color section to MED.INI, allows hi-lighting of patient name based on Flag1.
 
01/15/2003, New version.   Added the printing of zero dollar amount items on the day sheet.  Changed the patient demographic screen to copy the patient's phone number to the billing page.
 
01/11/2003, Very minor changes.  Ambulance users, send me an email.  Backup the data on your server and the workstation prior to installing any update.  Network users, be sure that the server is on and accessible.  The setup.exe program will attempt to update the data on your local drive if the server cannot be found.  Always backup the data on the server and the workstation.  Think, updates always contain the possibility of data problems.
 
01/01/2003, Happy new year.  New version, read 12/12/2002 news.  One user could not change the font size and attribute.  I have changed the help file to provide more information.  When using fonts for the insurance forms, leave the printer name blank and put an '8' in the LPI for 8 lines per inch or leave the LPI column empty for 6 lines per inch.  The line height has been increased for the Insurance Forms.  Some printers print the letter 'Q' larger the then previous line height would permit.
 
12/12/2002, New version added, this is a major update. BACKUP YOUR DATA.  Many files will be changed, Patient, Patient Insurance, Provider, Referring Physicians, Charge Header and Speciality codes.  These changes are necessary for the move to the ANSI x12 4010 electronic layout.  I am working on the ansi change as mandated by the HIPAA regulations.
 
11/30/2002,  Added the ability to print insurance claims by a range of provider codes, or one provider if the beginning and ending code are the same.  This is a temporary test, this feature may be deleted in a future release.  Your comments will determine if this ability is keep.  Again, READ the SETUP instructions.  Leave the beginning and ending provider codes (not the provider name, but the provider code) blank to print all providers, the default method of printing insurance claims for the last 21 years.
 
11/27/2002, This pops up every so often, READ the SETUP instructions.  You must edit the MED.INI file if you use Windows 2000 or Windows NT.  You must edit the MED.INI file if you install the program in a directory other then C:\MEDBILL.  You must edit the MED.INI file if you do not have a C:\WINDOWS\TEMP directory.  Most users do not need to edit the MED.INI file. You should read the first dozen pages of the users manual, MANUAL.PDF.  The users manual can be found at the START->PROGRAMS->MEDICALBILLING->MANUAL.  The users manual is an ADOBE document.  You may download ADOBE by following the link on this page.  The program has extensive on-line help, use the Help Buttons or the F1 key.
 
11/02/2002.  There was a problem with the un-registered password.  The free program is limited to 50 uses of the program or 30 Days. The password should pop up after this time. Sorry, some users had the request for password pop up on the first usage.  This has been fixed.
 
10/25/2002,The backup and restore is now working in CLAIMS.  There is a problem with the lookup buttons.  The lookup buttons on the update UB92 screen are global.  This means that it is possible to click on the wrong button/entry field combination.  Read the help file and use caution with the UB92 lookup buttons.
 
10/22/2002,Claims has gone commerical.  Initial help files and users manual has been added. The help and maunal will be improved in the coming weeks.  Claims is limited to 15 days or 50 claims.  You will need to purchase the program to go beyond the days and claim limit.
 
10/05/2002, Paid has been changed to CLAIMS.  This program is still in development.  There is on screen help for the 1500.  Work is on going. A new setup program has been created, the SETUP.EXE program will install the program in the \CLAIMS directory.  Previous users may copy the *.tps files to this new directory.
 
10/05/2002, Your TPSFIX.EXE file may be corrupt.  Download the latest version.  The Setup.exe file had a problem in mid September.  If in doubt, download the latest version.
 
09/30/2002, A new program has been added.  PAID allows you to enter information directly onto 1500's and UB92's.  The program will be free, no limits for a few weeks.  The current version of PAID contains very limited help and no users manual.  The program will cost $50.00 when the users manual and help files are added.  Users who download and register prior to the end of the free promotion will receive the users manual and help files (PDF format).  Comments and suggested changes should be sent to Billing Medical

07/28/2001, I have moved all files to the default installation directory, this is normally c:\medbill.  The temppath (temp directory) is still used.  Remember, W2k and WinNT, your datapath and temppath must be changed.  This move means that the TMED.INI file, used to save screen size and position has been moved from the \windows directory.  You have two choices, move the TMED.INI from windows to your installation directory or let the program save the windows size and position as you use the program.  I wanted all the program files to be in one directory. 
 
07/20/2002, A Hipaa notice and authorization screen has been added to the patient update screen.  You may enter information about privacy notifications, authorization of release of information.  A report may be printed.  All insurance forms are forced to upper case.  

07/17/2002, Additional security options have been added.  You must read the SECURITY.PDF file.  Everyone must be a supervisor or some functions will not work.  It is impossible to set permissions without being a user with supervisor rights.  A user with supervisor rights can change permissions for all other users.  

07/13/2002, I have added a lock screen button to the main menu.  You may lock your computer screen any time you wish.  The computer is not locked.  The MED program screen is blanked and the login/password screen appears.  You must re-enter your password.  MED tracks all logins, successful and aborted logins.  These changes should bring MED into compliance with the HIPAA security requirements.  

07/08/2002, Security has been added the to many of the options on the MAINT menu. You must have supervisor rights to use most of the MAINT menu options. More information will be provided as I complete the addition of user set-able security.  HIPPA, file an extension for compliance.  A new web page will be added in a few weeks with a great deal of HIPPA information.  The Batch pay screen has been changed, read the online help.  

06/17/2002, Minor changes.  If you have trouble with the login/password; test the sample data (start -> programs -> Medical Billing -> Sample.  If the login and password works with the sample program then you can copy the dssw2.tps file from the \medbill\sample directory to \medbill.  All data on the 1500 and UB92's are now translated to upper case at print time.  A transaction log has been added. See the online help for instructions.  The transaction log allows you to track account changes by login.

05/18/2002, Some users have had problems with the user and password.  A new version is ready.  Please call if you need assistance.  I have added additional support for Lindsay Technical Consultants for NSF files.

04/28/2002, Security Login has been added.  The initial login is USER and the password is PASSWORD.  You should change this password and add other users. Do not forget your password...  Don't forget to checkout the QUERY program.  You can create simple AD-HOC searches and print many user defined lists.  For example you can print all the patients over 65 and print mailing labels for these patients.

04/10/2002, Minor changes, increase patient account screen amounts to allow greater then 99,999.99 amounts.  Print insurance filed report for individual patients will print secondary insurance filed information.

03/30/2002, New version, uses new DLL's.  Minor changes and fixes to a few screens. IF you install to a drive other then C:, edit the med.ini file.  Failure to edit the med.ini causes many problems.  The ini file is used to customize your installation.  Read the Setup.txt file in HELP.

3/16/2002, Sample data has been repaired.  Remember to change the med.ini file in the \medbill directory and the \medbill\sample directory to reflect your drive.  The default drive is C:  Also, Windows NT and Windows 2000 may not have a \Windows\temp directory.  You may need to change the location of the temporary directory in the med.ini file.

02/24/2002, Allow 6 characters for unit of service.  The HCFA will only use the first three, UB92's need the additional characters.  This will update the Account file, backup your data.

2/22/2001, Changes to UB92s, addition of submitter id numbers, payer id and receiver type to the insurance company information. Read the user manual for information concerning UB92's and electronic filing.

12/16/2001, Changes to the NSF file.  You may have a carriage return/line feed placed at the end of each record, a check box has been added to NSF screen.  Some clearing houses require a CRLF, most carriers do not want the CRLF.  Added a check box to the insurance setup screen to ignore the account type.  Check this box to print the insurance form type you wish, the account type will be ignored.  Additional error checking has been added to the NSF file.

12/04/2001.  Major update.  Backup your data prior to installing the update.  The account.tps and office.tps file will be converted to a new format.  You set a charge type for ticket of UB92 or 1500.  The HCFA 1500 is the default type for a charge ticket.  I am in final testing with "The Health Information Network", THINS.  We will be able to file electronically, both UB92 and 1500's, with many national carriers.  Check out the THINS web site for information, go to the Vendor corner to see a payer list.  Read the users manual for additional information (setup, UB92 and entering Charges).  The download page contains a utility FIXUB92.EXE that will change all charge types to UB92.  HCFA 1500 users do not need this and users with multiple charge types should not use this utility.

11/20/2001, There is a problem with UB92 unit of service, box 46.  The Insurance layout file (on the Maint Menu) needs to be changed.  Change the Length of all UBO:BOX46[x] to 6 from the default 4.  I have a small patch that will automate this process. Send an email requesting the program to fix the box46 problem. Request box46 fix.  New program loaded, minor fixes to the sample data base and some changes to the NSF file.<

11/08/2001, I will be adding security support to MED.  You will need to login with a valid password to start the program.  Users rights may be limited by the program administrator. (A single user will be both the administrator/super user and user). You will be able to set view only operation based on the login name.  The registration and use of future versions of the free program MAY change.  I am thinking of limiting the program to either 10 patients or to 30/60 days.  I am interested in your comments.  Changes will not affect the current version of the program but may change  future upgrades.

11/04/2001, Added Multi Charge, print the new users manual, check the on-line help files.Ub92 charge lines are now bundled, that is, charges with the same revenue code or same HCPS code will be placed on the same charge line.  This is a major change for UB92, I am not sure that is fully de-bugged.  UB92 customers, let me know what you think of this.  I have also added the ability to print patient statements by account type.  You can leave this blank to print all account types.  Many users have asked for these UB92 changes and the ability to automatically add multiple charge lines.  These changes may be reversed, based on user feedback.

11/02/2001, Changes to custom insurance and statement layouts.  Please contact me if you are having trouble with aligning the insurance forms.  There may be a very small charge to setup your insurance layout for your printer.

10/23/2001, Changed the default font on the HCFA and UB92 to 'Courier New' this should help with printing UB92's.  I recommend using the generic/text only printer for HCFA's.  Some printers have trouble with the UB92.  A few minor changes.

10/12/2001, Problem with UFORM.TPS file.  If you are having trouble with the UB92 files, (Unable to convert UFORM), download the 10/12/2001 version of setup.exe.

09/11/2001,  Check the FAQ page.  Many people with Windows 2000 and Windows NT are having trouble with the IFORM.TPS file.  The temppath variable in the MED.INI file needs to be changed to \WINNT\TEMP.

09/07/2001, The win32/funlove virus was on the SETUP.EXE file that was loaded on 8/28/2001.  This has been fixed.

9/03/2001, Changes to custom superbill.  You may add an entry in the Statement section, SUPERBILL=CUSTOM, or any other statement form. This will allow you to create your own custom superbill/ticket.  The users manual has not been updated with the new instructions.  I have turned on some debug flags.  You may see an error message concerning array out of limits.  This is caused by miss use of the insurance layout or statement layout.  Please email me with any array or other error messages.

08/28/2001, The price will increase to $495 the first week in September.

08/22/2001, Added conversion for UCHARGE.TPS, this was omitted in the 08/11 upgrade. Will now print the charge description on the statement for the LINK code.  Updates the 'bill to name' automatically when inserting a new patient. This had been removed but is being added back due to popular demand.

08/11/2001, Added additional Diagnosis fields to Update UB92 Charge header screen, boxes 71 to 75.  Most people will not need this update.

08/07/2001, Minor fix to insurance print program.  Patients without insurance were printing when a single insurance company was requested.

08/02/2001, Fix to charges screen.  The charge should be automatically filled when a valid cpt code is entered. Added a link code to the cpt file.  The link code will allow UB92 users to create codes that will not be printed in box 44 of the UB92.

07/24/2001, Please read the users manual or print the setup.txt file. Upgrade users need to modify the  MED.INI file.  Use NOTEPAD to edit the \MEDBILL\MED.INI file.  You may want to add these lines. The FIXNAMES section controls the automatic update of patient, patient insurance, provider and referring physician names from the Last, First and MI fields.  Normally you want the Patient and Pins files automatically updated but the Provider and Refdoc not to be updated.

07/07/2001, Updated the users manual.  Added the ability to set the lines per inch to 8 LPI for UB92 printing.  The UB92 layout would not work with Laser printers.  Download the latest SETUP.EXE,  print the report for each of your current Insurance Layout files.  Delete the MREPORT.TPS in your data directory.  Run the Setup program. Redo you changes to the Ins Layout file.  The UB92L layout is the layout for Laser printers.

4/22/2001, Improved sample data has been included SETUP.EXE.  Download and install SETUP.EXE. Goto START -> PROGRAMS -> Medical Billing -> Sample Medical Billing. The sample data file has about 30 patients with charges and payments.  You may test the insurance, statements and reports.  Most of the data is in a date range of September 2000 to December 2000.

4/19/2001, Major update, backup your data, PLEASE BE SURE TO HAVE A VERIFIED DATA BACKUP.  This update adds the ability to upgrade files automatically, UB92 improvements and a few minor changes.  This will become a BASELINE update.  All future updates will require this version to have been installed.  Future changes will allow you to modify the built in reports and allow import/export of your data files.  The help files and user manual has not changed, there will be an updated manual available in a few weeks.

04/01/2001UB92, Hcfa 1450 support has been added to MED.  The UB92 is used for Hospital and certain type of practices.  This update will modify many of your existing files.  Be sure to backup your files using a verified backup.  Do not depend on the built in backup program.  Contact me if you receive any errors, there will be no charge for the support call.

02/22/2001, NACPP Medical Executive Practice Manager offered, check www.nacpp.org for details.  I have added the ability to edit the insurance forms, go to the Maint Menu, select Last Ins File.  You may edit the claim(s) and reprint all or any one claim.  Price increase, registered users with a password starting with 'S' will not have to purchase the program, users with a password starting with 'M' may purchase the un-limited version for $150.00.  The limited version is limited to 50 patients.  I will provide a password, as needed to all 'S' users.  Email me when you hit more then 50 patients and I will send the unlock password.  Phone support will now cost, $30.00 per incident (as many calls as it takes to solve a problem). You may purchase 90 days of support for $180. 00. Check the order page for other support options.

Please notify me of any error 37 or 47, these may appear after creating an NSF file.   This is a very minor update.  Most people will not need to update.

01/24/2001, Very minor update.  One user has a problem with error 37's after creating an NSF file. The same user has to re-boot in order to start the MED program.  Please send an email if you are having these problems.  The Update changes the method used to close the files after creating an NSF file.

01/18/2001, ZIPS.TPS was corrupt or missing on some dowloads, you may copy the ZIPS.TPS file from the \medbill\sample directory or download ZIPS.ZIP, unzip the file and copy it to the \Medbill directory (assuming \Medbill is your data directory).

12/11/2000. Changed window re-size/position scheme.  A temporary file, \windows\tmed.ini contains the saved size and position of most windows. Delete \windows\tmed.ini to restore all windows to the default size and position.  Split installation file.  You may download FSETUP, three files, if you have trouble downloading the very large Setup.exe.

12/03/2000.  New version has been loaded.  This version fixes some minor problems with some of the data entry screens.  I have used the new version of Softvelocity 5.5 compiler.  This is a major version upgrade. The DLL's, dynamic link libraries have been changed.  The Query program has been modified by use of a new Query By Example library.  A potential problem with the Charges and Payments has been repaired (it was possible to lose a payment line item when using the charge entry form, this was a very rare occurrence).  Backup your data, the *.tps files.  During  the installation a dialog box will permit you to backup replaced files, PLEASE CHECK YES, BACKUP REPLACED FILES.

11/04/2000: Soon, people who have completed the registration form will receive a login and password. You will not need to complete the form to download updates. Currently, there are no updates. By mid-December, the program will be taken off free status, current users will be able to download updates for many months. The current program has no known bugs. I will start selling the program for a very small fee, somewhere between $100 and $200. Support contracts and per incident support fees will be posted. I think the yearly support fee will be in the range of $200 to $300. Per incident fees will be $50.00. I will create a FAQ page. You probably will not need support. There may be a reduced fee for users who have PC-Anywhere version 8 or greater and an internet connection. Please feel free to comment on these coming changes. You may print labels for Referring Physicians, Insurance Companies, Hospitals and three types of Patient labels.

NSF v 3.01 support has been added. You may use the nsf file to submit to a clearing house. The NSF file creation process performs an edit check on the insurance file. It would be a good idea to run the nsf portion when printing insurance claims, check the NSF box on the setup insurance form screen.

Ad-hoc query capability has been added, 07/31/2000. Download query.exe. Query will allow 'Filter' type searches on the data base. Help files will be added in a few weeks. The History file is in testing, you will be able to move patients with a zero balance to a history file, should be ready mid September.

03/22/2000, Electronic Remittance Advice has been added. ERA supports nsf version 2.01 The Med and Maint program have been combined. Med contains the office setup and file maintenance routines, Maint is no longer needed and should be deleted. The Charges and Payment screens have been changed, use the arrow and tab keys to move between fields. The ERA program will be upgraded to version 3.x in a few weeks. Use caution when using the ERA, check your reports.
